  The original National Identity Card with a certified copy by the Commissioner Department of Registration of Persons at C45, Keppetipola Mawatha ,Colombo 05 (Telephone : 011-2583199, 011- 2508022 Fax: 011-2593634 E-mail : commrpd@sltnet.lk Web site : http://www.rpd.gov.lk ) should be provided. The Certified copy should be countersigned by the Consular Division of the Ministry of Foreign Affairs of Sri Lanka.
